Campus Health Connect - TTUHSC

Lometa ISD has a partnership with Texas Tech University Health Sciences Center to provide non-emergent student health needs in-house. 

The program will allow Lometa ISD staff to facilitate a virtual, video connection between TTUHSC personnel and Lometa ISD students in need of non-emergency medical care.

In order for students to participate, parents must take care of the required Forms below.

Each of the forms on the form link below must be filled out prior to a virtual office visit.  It would be helpful to fill out the forms at the parents' earliest convenience so that they are ready when a student needs a doctor visit.  Parents will be contacted for permission to treat before any virtual doctor visit. Having the forms prepared ahead of time will facilitate the process. 

The forms are electronically signed, but each form may require you to verify your activity by clicking an email link. Be prepared to move back and forth from the form page to your email.  You may use a phone, tablet or computer for the process. 

The Charity Care program form may allow students to be treated without the $20.00 charge.  Any student who has qualified for "Free or Reduced Lunch" should have the "NSLP" choice selected. Other qualifying subsidies may also be checked.  The image below shows what you will see on the Charity Care form.
